Job Description

About The Job:

The clinical nurse leader is a key nursing leadership position providing leadership and mentorship to practice setting staff and leads initiatives to ensure quality care outcomes for the patient and their families. The clinical nurse leader oversees the plan of care for patients and families and monitors/facilitates the flow of patients within the practice setting, and works with the multidisciplinary team to ensure care is integrated; person and family centered, based on best practice, and is effective and efficient. The clinical nurse leader models and implements evidence-based strategies to enhance teamwork, communication, autonomous clinical practice, critical thinking, best clinical practices, and drives excellence in patient safety standards and quality outcomes in collaboration/partnership with frontline multidisciplinary team members. Reporting to the nurse manager, the clinical nurse lead collaborates with the nurse manager/clinical nurse educator to ensure a high standard of care is delivered and quality initiatives are implemented and maintained, in compliance with organizational policies/procedures.

 

 

 

Duties will include:

  • provide leadership, mentorship, guidance and evaluate front-line multidisciplinary team members to ensure comprehensive and integrated care planning occurs for patients to achieve outcomes that are of high quality and safe;
  • provide clinical nurse leadership and direction by ensuring patient care is delivered according to professional nursing standards and in accordance with facility/provincial policies and procedures;
  • ensure that a comprehensive individualized nursing care plan exists for each patient in the practice setting by working in collaboration with all members of the multidisciplinary team. Participate in reviewing, revising, delegating and formulating the plan of care following ongoing patient assessment by multidisciplinary team members;
  • act as a professional role model, mentor and change agent for staff by demonstrating clinical expertise and evidence of ongoing professional development in their clinical specialty; promote and apply professional nursing standards and codes of conduct;
  • assesses risk management initiatives to ensure patient and team safety;
  • identify educational requirements for the multidisciplinary team members in collaboration with the clinical nurse educator and specific to the practice setting; support and educate staff clinically at the point of care;
  • participate in staffing and orientation of new staff; provide feedback on preceptoring/mentoring relationships, new staff integration in collaboration with the nurse manager and clinical nurse educator and identify supports and assistance for these relationships as required;
  • apply the principles and activities of the accreditation process, pursue accreditation standard of services delivery; facilitate and promote quality improvement;
  • review patients’ charts/conduct regular audits and ensure staff are meeting Health PEI and facility documentation practices, policies, standards and guidelines, and follow up on audit results with nurse manager and / or staff to ensure adherence;
  • utilize new technology and evaluate its impact on staff, patients, families and overall best practices, and address/report concerns to the nurse manager;
  • serve as a direct care provider for complex situations, as appropriate;
  • participate in health care system and hospital-wide initiatives which support the strategic direction of the organization;
  • other related duties.

 

 

 

Minimum Qualifications:

  • Baccalaureate degree in nursing. Applicants with a diploma in nursing and currently enrolled in or in the process of enrolling in a BN degree program will be considered.
  • Registered or eligible for registration with the College of Registered Nurses of Prince Edward Island;
  • Extensive experience in the area of job assignment; ability to provide input into relevant policies, standards and guidelines
  • Willing to undertake further leadership education within first year of accepting the position;
  • Strong interpersonal skills; ability to communicate verbally and in writing and collaborate effectively with staff, health professionals, patients and their families; ability to exercise diplomacy, tact and judgment in dealing with difficult and complex situations;
  • Ability to participate in and complete unit/facility clinical audits;
  • Understanding of QIA process, Accreditation Canada standards, evidence-based practices and ability to support and implement policies and standards in facility/unit to ensure quality person and family centered care;
  • Ability to work independently and as part of a team, while respecting professional boundaries, cultural differences and team decisions;
  • Ability to develop, coordinate, lead and evaluate special projects and program changes;
  • Knowledge and broad understanding of the health care system on a provincial basis;
  • Knowledge of current trends and issues in health care that enable development of options and recommendations;
  • Good previous work and attendance record;
  • Experience using computer program and software (as noted below); e.g., e-mail, Microsoft office, word perfect, internet, CIS, etc.

 

 

 

Other Qualifications

  • Certification in Psychiatric Mental Health Nursing with Canadian Nurses Association is preferred.
  • Non-Violent Crisis Intervention; Gentle Persuasive Approach; Advance Code White; and Suicide Risk Assessment will be considered an asset;
