How Registered Nurses (RN) support our team Renal Clinic at Grand River Hospital:
The successful candidate’s primary focus will support early management of Chronic Kidney Disease (CKD) which includes CKD identification, prevention and the concepts of patient self-care management, and facilitating a continuum of CKD patient education to the stage of informed decision making. Responsibilities also include patient screening, preparation and promotion of independent modalities. These responsibilities will be in collaboration with the Nephrologists, the Allied Health team and the CKD clinic nurses within a mentorship environment. The candidate will contribute to ensuring there is a safe environment for patients, staff and visitors.
